How CalmTide Turns Apple Watch Data Into a Cortisol Trend

Your Apple Watch has no cortisol sensor, and CalmTide never pretends otherwise. What it does have is a night's worth of readings that move with cortisol — and a model that turns them into one number each morning, a baseline over weeks, and a direction. This is how that works, reading by reading.

We've written before about why no smartwatch can measure cortisol. That article ends with a promise: the proxies your Apple Watch records are still worth reading, as long as you're honest about what they are. This one is the follow-through. It walks through exactly what CalmTide pulls from the watch, how each reading nudges the daily cortisol estimate, how those daily estimates become a trend, and — just as important — the cases where CalmTide deliberately leaves a gap instead of guessing.

If you only take one thing away: a single day's number is a weather report. The trend is the climate. CalmTide is built to show you the second thing.

The one sentence to keep in mind

CalmTide estimates cortisol load. It does not measure cortisol. Every number in the app is a research-based inference from signals your Apple Watch already records, computed on your iPhone. Cortisol itself is only ever read from saliva, blood or hair in a lab.

That distinction isn't a legal footnote — it decides how you should read the screen. A lab value tells you what your cortisol was at one moment. A wrist estimate tells you which way your stress physiology is leaning, every day, for free, without a needle. Different tool, different job.

What the Apple Watch actually hands over

CalmTide reads from Apple Health, and the readings that matter for cortisol almost all come from the hours you're asleep. That's not an accident: overnight is when your body is least disturbed by coffee, meetings and stairs, so the signals are cleanest — and it's the window where the research linking them to the HPA axis was mostly done.

Two details in that table do more work than they look like. The HRV is overnight HRV — not an all-day average, which runs roughly a third lower and would make a normal sleeper look chronically stressed. And the daylight is morning daylight, not the day's total, because an evening walk does nothing for a factor that's about anchoring your wake time.

Step one: last night becomes today's number

Every morning, CalmTide folds the readings above into a single estimated cortisol load on a 0–100 scale. The mechanics are deliberately simple enough to explain in a paragraph, because a number you can't explain isn't one you can act on.

The estimate starts at a neutral 50. Each signal then either adds load or takes it away, and only if it was actually recorded. Short sleep pushes the number up in steps — under seven hours, and again under six — while eight hours or more pulls it down. Overnight HRV works the same way in the other direction: above 60 ms eases the load, below 40 ms adds to it, below 25 ms adds a lot. A resting heart rate above 75 bpm or a resting respiratory rate above 18 breaths a minute each add a little. Every workout contributes according to its intensity zone and duration, so an easy hour subtracts and a hard interval session adds. Mindful minutes subtract, up to a cap. And hitting your morning-light target takes a few points off.

The result lands in one of four bands — Low, Moderate, Elevated or High — and the neutral 50 sits at the top of Moderate on purpose. A day where nothing was measured is a day CalmTide knows nothing about, so it must not read as a verdict.

Step two: days become a trend

A daily estimate on its own is easy to over-read. Wake up after one bad night and you'll see an Elevated day — which is true, and also not very interesting, because you already knew you slept badly. The useful question is whether Elevated days are becoming more common than they were a month ago.

So CalmTide keeps every daily estimate and rolls them up, week by week, into a baseline: an estimated resting level that moves slowly, isn't thrown by a single bad night, and gives each new week something to be compared against. On the Trend screen you'll always see where that baseline sits now, how far it has moved since you started, and a plain-language direction — falling, steady or creeping up. That summary is free.

Alongside it, once a week, CalmTide scores the week as a whole with the HPA Balance Score. This uses the same inputs but weights them by how strongly the research ties each one to HPA-axis regulation: exercise dose 30%, HRV 25%, sleep 20%, mindfulness 15%, morning rhythm 10%. Since version 1.6 the sleep component also includes sleep regularity, not just duration — an irregular schedule disturbs the cortisol rhythm independently of how long you slept (Windred et al., 2024). The weekly score is the slow, honest signal; the daily estimate is the fast, noisy one. You want both, and you want to know which is which.

The forecast is worth a sentence, because "forecast" is a word that invites over-claiming. It's an explainable projection built from three things — the momentum of your recent estimates, a gentle pull back toward your personal baseline, and lingering pressure from your training load — and it labels itself with a confidence level. It answers one question only: if I keep doing what I've been doing, where is my load heading?

What CalmTide refuses to guess

The part of the model we're proudest of isn't a feature. It's a rule, and version 1.6 was mostly about enforcing it everywhere: what CalmTide couldn't measure leaves a gap in the picture, never a zero in your score.

The failure mode this prevents is subtle and very common in wearable apps. Leave your Apple Watch on the charger one night and the naive approach records zero hours of sleep — which the model then scores as a catastrophic night, spikes your cortisol estimate, and drags your weekly score down for a night you may have slept perfectly well. CalmTide treats a missing sleep sample as unknown. The sleep term simply drops out. The same goes for a night with no overnight HRV, for deep sleep on an iPhone-only setup that can't record stages, and for a morning where daylight was never measured. None of them count against you, and none of them count for you either.

Every score also carries a data-confidence indicator — free, on every tier — that counts how many recent days actually had signal behind them and how many nights went unrecorded. When a number looks surprising, check it before you react: a sparse week earns "low confidence" rather than a confident-looking verdict, and until there's enough data the app says "not enough data yet" instead of inventing a worst case.

Reading the trend well: ignore any single day that follows an unmeasured night. Look at the baseline direction over two to three weeks. If it's creeping up, the weekly readout will usually point to one of the usual suspects — stacked hard sessions, a run of short nights, or alcohol, caffeine or late meals — before you'd have noticed it yourself.

A worked week

Say your baseline has been sitting comfortably in Moderate for a month. Monday you race a 10K and sleep six hours; the daily estimate reads Elevated, driven by a hard session and short sleep. Tuesday you skip the watch overnight; that estimate stays close to neutral because there's nothing to score, and the confidence indicator quietly logs one more unrecorded night. Wednesday through Friday you sleep seven and a half hours, your overnight HRV recovers to its usual range and you log two easy Zone 2 sessions and ten mindful minutes; the estimates drift back into Moderate. By Sunday the weekly HPA Balance Score is a touch lower than last week, the baseline direction still reads "steady", and the weekly readout names Monday's race as the single biggest mover. Nothing alarming happened — which is exactly the reassurance a trend gives you and a single day never can.

What happens on a night I don't wear my Apple Watch?

That night is treated as unknown, not as bad. A missing sleep sample or a missing overnight HRV reading is left neutral, so it never inflates your cortisol estimate. Your scores also carry a data-confidence indicator so you can see how much of the recent picture was actually measured.

How long does it take to see a cortisol trend in CalmTide?
